Cancellation

We require 48 hrs notice for cancelled appointments

Missed or cancelled appointments will incur a fee of $100.

Third-Party Insurance

Although we do our best to obtain as much information on our patients insurance coverage, it is the patients responsibility to know and understand their insurance coverage, limits and restrictions. Privacy issues sometimes prevent our office from obtaining information from a clients insurance carrier so we ask our clients to do their own due diligence with regards to insurance coverage.
